The application portfolio is a recording and compilation of documents demonstrating competency in the knowledge and skills specifically related to the functions of a certified.  It consists of:

• The Application
• Supervisor Evaluation Form
• Work Experience Form
• Colleague Evaluation Forms

The FCB reserves the right to research all experience, documentation and recommendations.  Additional information will be used only to further evaluate an applicantand will be held confidential.
